- Facility Tours and Pre-Admission Consultations
We invite you to tour our facility, where we will explain our services and eligibility requirements. After you’ve had a chance to experience the atmosphere of the facility, we will be happy to discuss your specific needs with you.
- Application for Service
- If you wish to use our services, your application will constitute a formal contract.
- Assessment
- Our Child Development Support Manager will consult with you to determine the specific support each child needs. In addition, a nurse will discuss the details of their care with you.
- Development of Individualized Support Plans
- Based on the discussions we have with parents, our consultation support staff will create an Individualized Support Plan tailored to each child using the Service Utilization Plan provided by them.
- Service Launch
- We will launch the service as planned.

Who can use this service?

This program is for children who hold a recipient certificate for severe physical and mental disabilities or medical care.
It is available for children aged 0 to 18.

What steps do I need to take to use this service?

First, you must apply for a beneficiary certificate at your local municipal office.
After that, you will have an interview and sign a contract at our facility, and service will begin once an individual support plan has been created.
